Bloom Camp Liability Waiver, Payment Policy, and Release of Liability

Bloom Camp is a program operated by Bloom Club LLC, located in Hollywood, Florida.

By registering my child for Bloom Camp, I acknowledge, understand, and agree to the following terms:
1. Nature of the Program
I understand that Bloom Camp is an indoor and outdoor day camp program operated at Bloom Club’s location in Hollywood, Florida, and may include activities in the facility, in the backyard, and during supervised walks or visits to nearby parks and outdoor areas.
Camp activities may include, without limitation, games, sports, creative activities, movement-based activities, playground activities, outdoor play, water play, use of recreational equipment, and interaction with other children.
2. Assumption of Risk
I understand that participation in Bloom Camp activities involves inherent and unavoidable risks, including but not limited to falls, collisions, injuries during physical activity, playground-related incidents, dehydration, heat exposure, allergic reactions, insect bites, illness, and injuries arising from interaction with equipment, outdoor spaces, or other children.
I voluntarily and knowingly assume all risks associated with my child’s participation in Bloom Camp, including activities conducted indoors, in the backyard, and during supervised walks or park visits.
3. Release of Liability
To the fullest extent permitted by law, I hereby release, waive, discharge, and hold harmless Bloom Club LLC, doing business as Bloom Camp, and its owners, members, managers, directors, officers, employees, staff members, counselors, contractors, volunteers, agents, representatives, successors, and assigns from any and all liability, claims, demands, damages, losses, costs, or causes of action arising out of or related to any injury, illness, loss, damage, or expense that may occur in connection with my child’s participation in Bloom Camp activities, attendance at camp, use of the indoor facility, backyard, playground or park areas, and supervised walks or outings.
4. Medical Authorization
I authorize Bloom Camp staff to provide basic first aid and to obtain or arrange emergency medical care for my child if I cannot be reached or if immediate action is reasonably necessary.
I understand that Bloom Camp staff will make reasonable efforts to contact me or the emergency contact listed on the registration form before obtaining medical treatment when circumstances permit.
I agree to be solely responsible for any and all medical expenses incurred on behalf of my child.
5. Health Information
I certify that the information I provide regarding my child’s allergies, medical conditions, medications, behavioral concerns, developmental needs, and special needs is accurate, complete, and current.
I agree to inform Bloom Camp promptly of any changes in my child’s health condition, medications, allergies, behavioral needs, or other relevant information.
I understand that Bloom Camp reserves the right to deny, limit, or discontinue participation if my child’s needs exceed the camp’s staffing model, supervision capacity, or ability to safely accommodate the child in the program.
6. Pick-Up Authorization
I understand that my child will only be released to individuals listed by me as authorized pick-up persons on the registration form, unless otherwise approved by Bloom Camp.
Bloom Camp may request photo identification before releasing a child.
7. Camp Rules and Behavior
I understand that my child is expected to follow all camp rules, safety instructions, behavioral expectations, and directions from staff at all times.
Bloom Camp reserves the right to restrict participation, require early pick-up, suspend, or dismiss any child whose behavior is disruptive, unsafe, aggressive, inappropriate, or otherwise interferes with the safety or well-being of staff or other children.
No refund, credit, make-up day, transfer, or rescheduling will be issued in such cases.
8. Payment Policy / No Refund / No Transfer Policy
I understand and agree that each camp session consists of one (1) week, and that full payment for each registered session must be made no later than two (2) weeks before the start date of that session.
I understand and agree that all payments made to Bloom Camp, including deposits, registration fees, weekly tuition, daily fees, and any other amounts paid, are final, non-refundable, and non-transferable.
I further understand and agree that no refunds, credits, make-up days, transfers, rescheduling, or carryovers will be provided for missed camp days or sessions due to illness, vacation, schedule changes, family matters, behavioral dismissal, withdrawal, late arrival, early pick-up, emergencies, weather concerns, or for any other reason.
I understand that registration reserves staffing, space, scheduling, and operational commitments, and that my payment obligation is not conditioned on my child’s attendance. If payment is not received in full at least two (2) weeks before the start of the registered session, Bloom Camp reserves the right to cancel the child’s spot without refund or credit of any amounts previously paid, unless otherwise agreed in writing by Bloom Camp.
9. Personal Property
I understand that Bloom Camp is not responsible for lost, stolen, or damaged personal items, including clothing, toys, electronics, jewelry, bags, or other belongings brought to camp.
10. Photo and Video Release
I grant Bloom Camp and Bloom Club LLC permission to photograph, videotape, and otherwise record my child during camp activities and to use such images, video, or recordings for promotional, advertising, marketing, educational, editorial, and social media purposes, including on websites, printed materials, and digital platforms, without compensation or further approval.
11. Governing Law
This waiver and agreement shall be governed by the laws of the State of Florida.
12. Electronic Signature and Agreement
By signing the registration form electronically, I confirm that I am the parent or legal guardian of the child listed in the registration form and that I have read, understood, and voluntarily agree to this Liability Waiver, Payment Policy, and Release of Liability.
